The Client Interview

HWS Care provides personalized home care services, ensuring clients live safely, comfortably, and independently at home.

At HWS Care, we are proud of our dedicated home care team, committed to ensuring individuals maintain a high quality of life. Our tailored home care services are designed to promote safety, comfort, and independence within the client’s chosen residence.

We offer a comprehensive range of services, including personalized assistance with daily living activities and personal care, all customized to the client’s specific needs and and agreed upon by the client or their representative.

During the initial consultation, an HWS supervisor conducts a comprehensive assessment, which is used to create a Case Summary. This summary includes important details such as client information, emergency contacts, service plans, and schedules. Caregivers receive the Case Summary at the start of each case, and it is updated as necessary.

The Service Plan, developed in consultation with the client and their family or representative, outlines the services provided, taking into account factors like activity level, diet, medications, and treatments. The plan is regularly reviewed, at least annually, and can be adjusted based on the client’s needs or at their request. Any updates will be confirmed with the client, caregiver, and HWS supervisor.

Once the service plan is finalized, caregivers are scheduled based on the agreed-upon schedule, with flexibility to adjust as needed. All changes will be communicated to the client, caregiver, and HWS supervisor.

HWS provides a service rate based on the initial service plan, with a full list of charges available in theRates for Services document. Any billing changes are communicated in advance, and a signed client agreement is required before the start of services.
